CMC Vellore

To apply for the CMC Vellore PG Admissions 2006

(i) Fill in the online applic ation form (Complete all parts of the online form by selecting the appropriate links. i.e., personal details, course details, sponsorship details and experience details).

(ii) Select the payment mode.

If the payment mode is DD, get the DD for the amount as per the course selection.

" Please enclose Rs. 400 towards processing fee plus Rs.400 per MD/MS/ Diploma Course or Rs.750 for a higher specialty course. Also Refer page 13 of the Bulletin by clicking the Bulletin icon above for detailed information on fees."

(iii) Submit the application after ensuring that all parts of the online application form are complete.

Please complete your form by filling all the portions of your application. (i.e.)

Personal (Mandatory)

Course (Mandatory)

Sponsorship (Optional)

Experience (Optional)

Kindly ensure that you have completed all parts of the online application else your application will be invalid.

(PLEASE READ THIS CAREFULLY - Please ensure that all the details furnished by you are correct and final. Once the application form is submitted, the system will not allow you to modify or edit the details)

2. On successful submission of the online registration form,

(i) A CMCV2006 ID will appear.

(ii) This ID along with a password will be mailed to the email id specified by you during the registration.

(iii) Please use this user id and password to log on to this system for further access and Hall Ticket information.

3. Take a printout of the complete form report, which contains the important details of your application, by clicking on the printer icon on the left hand side top corner of the screen. This printout will indicate the required enclosures.

4. In the complete form report, paste your recent passport size photograph and sign in the appropriate box given. (This will be verified against a photo id and authenticated by the CMCV Representative/invigilator at the Test Centre on January 7, 2006)

Step b)

Click on the payment advice form and take a printout of the challan.

You can remit your payment in any one of the following methods:

1. Visit any ICICI Bank branch with the three part pay-in-slip, make the payment and get the pay-in-slip authenticated by ICICI Bank. Please mention ' FC-CMC-V ' on the Challan.

2. Send a DD drawn in favour of "CMC Vellore Association A/c" payable at State Bank of India (Branch Code 2203) Bagayam Branch or Central Bank of India (Branch Code M-10) Vellore, or any scheduled bank payable at Vellore.

3. CMC Vellore Association challans of the State Bank of India, Bagayam Branch.

4. Receipts of C.M.C. Vellore Accounts Department paid to the Credit of "Post graduate Selection Fund".

Step c)

1. Send the signed complete form report, Challan (or DD / Receipt) and necessary enclosures to: The Office of the Registrar, CMC, Thorapadi Post, Vellore - 632 002. You are requested to furnish your full postal address with the pin-code.

2. Mention your telephone number and e-mail address if available. Please keep a photocopy of the complete form report for your reference.
